Help!

I am trying to fill out my taxes, and realized I am missing a form.

Stupid state of NY. They gave me a refund last year (hooray!) but never sent me the 1099-G that I need to fill out my federal tax form for *this* year!

I know the amount I got paid. What I need to know is the Payer info: name, address, etc and the ID #.

So if there is someone else out there who got a state refund this past year, any help would be much appreciated.

Although it appears that you started before I did. I didn't start until 10 pm. And it seems that for some reason, my W-4 forms at work have an extra exemption, and thus not enough (not nearly enough) was withheld this year. Blech!
